What Is Blepharitis?
Blepharitis is a chronic inflammatory condition of the eyelid margins — the edges of the eyelids where the lashes grow. It affects the skin, lash follicles, and the tiny meibomian oil glands that keep the tear film healthy. Blepharitis is one of the most common eye conditions seen in optometry and is a leading cause of chronic dry eye, eyelid discomfort, and recurring styes and chalazia. While it is rarely vision-threatening on its own, it is highly persistent and progressive when left untreated.
What Causes Blepharitis?
Blepharitis has several underlying causes, which is why a proper clinical evaluation is essential before treatment. The most common causes include:
- Bacterial overgrowth — Staphylococcal bacteria colonize the eyelid margin and lash follicles, triggering inflammation, crusting, and eyelash loss over time.
- Demodex mite infestation — Microscopic mites (Demodex folliculorum and Demodex brevis) live naturally in hair follicles and sebaceous glands but become problematic in large numbers, causing chronic eyelid inflammation.
- Meibomian gland dysfunction (MGD) — When the oil glands in the eyelids become clogged or dysfunctional, the resulting poor-quality lipid layer triggers posterior blepharitis. MGD and blepharitis very frequently coexist and require treatment of both. Learn more on our Dry Eye Treatment page.
- Seborrheic dermatitis — This skin condition, which also affects the scalp and face, is strongly associated with anterior blepharitis.
- Rosacea — Ocular rosacea frequently manifests as chronic blepharitis, particularly the posterior (meibomian) form.

The Symptoms of Blepharitis
Blepharitis symptoms vary depending on the type and severity, but commonly include:
- Red, swollen, or irritated eyelid margins
- Crusty or greasy flakes at the base of the eyelashes, especially upon waking

- Burning, stinging, or itching sensation in the eyes

Treatment Options for Blepharitis
Blepharitis is a chronic condition, but with the right combination of in-office treatment and consistent at-home care, symptoms can be dramatically reduced and long-term eyelid health preserved. At Eye Care Center LTD, we tailor treatment to the type and severity of blepharitis identified during your examination. Treatment options include:- BlephEx In-Office Eyelid Debridement – BlephEx is a professional-grade, in-office lid cleaning procedure using a small handheld instrument with a medical-grade microsponge that rotates along the eyelid margin to remove bacterial biofilm, Demodex waste, and accumulated debris that at-home cleaning cannot reach. Most patients experience immediate improvement in comfort. A series of treatments is typically recommended for moderate to severe cases, followed by periodic maintenance sessions every 4 to 6 months.
- Daily Eyelid Hygiene with Medical-Grade Cleansers – Hypochlorous acid (HOCL) eyelid sprays have strong evidence for reducing bacterial load and eyelid inflammation. Tea tree oil-based lid scrubs are particularly effective for Demodex-related blepharitis. We recommend specific products based on your blepharitis type and provide instruction on proper technique to maximize effectiveness.
- Warm Compresses – Applying consistent heat to the eyelids softens thickened meibomian gland secretions, improving oil flow and reducing the inflammatory buildup that drives posterior blepharitis. - Oral Doxycycline (Low-Dose Anti-Inflammatory) – At sub-antibiotic doses, doxycycline reduces eyelid inflammation, improves meibum quality, and addresses the meibomian gland dysfunction that drives posterior blepharitis. It is particularly effective for rosacea-associated blepharitis and is used in combination with in-office procedures for moderate to severe cases.
- Demodex-Targeted Treatments – When Demodex mite infestation is identified, targeted treatments including in-office lotilaner (Xdemvy) therapy or tea tree oil-based lid scrubs are incorporated. Demodex-related blepharitis requires specific management as standard lid hygiene alone is insufficient to control mite populations.
- Intense Pulsed Light (IPL) Therapy – For patients with posterior blepharitis associated with meibomian gland dysfunction and rosacea, IPL therapy delivers targeted light energy to the eyelid vasculature, reducing inflammation, liquefying thickened meibum, and improving gland function. IPL has a strong evidence base for this patient population and is available at our practice.
- Short-Term Topical Corticosteroids – In cases with significant acute inflammation, a short course of low-potency topical steroid drops or ointment may be used to break the inflammatory cycle before transitioning to long-term maintenance therapies.
- Omega-3 Fatty Acid Supplementation – High-quality omega-3s at therapeutic doses (2,000 to 3,000 mg EPA+DHA daily in re-esterified triglyceride form) improve meibum quality and reduce eyelid inflammation over six to twelve weeks of consistent use.
